EMT Course Instructions

EMT Course Commencement

Within 10 days of the EMT course commencement, instructors submit the following:

Course Notification Report (PDF)
Course Summary Roster (PDF)
EMT Initial Application (PDF) Instructors please be aware that a state criminal background check from the state (s) of residency of the past 5 years should accompany application.

EMT Course Completion

Within two (2) weeks of the EMT course completion, instructors submit the following:

Course Summary Report (PDF) (Please note anyone who withdrew or failed the course
Course Summary Roster (PDF) for only those students requesting to take the NREMT-FR Written Exam
Checklist Items for Each Student, including the following:
- Student Eligibility Form (PDF) for each student
- $30.00 Certification Fee (certified check or money order)
- FBI Background Check
- Healthcare Provider CPR Card
- HIV/AIDS Certificate
- Practical Skills Sheet Results

Annual Summary Report

No later than July 31, of each year the EMS-TEI Course Coordinator will submit the EMS TEI Annual Summary Report (PDF) Form.

If no courses were taught during the last reporting period, file an Annual Summary Report stating that no courses were taught during the reporting period.
