Within 10 days of the FR course commencement, instructors submit the following:

• Course Notification Report
• Course Summary Roster
  • Agency Name, Course Number, Student Name & Student SSN are all that is needed at the beginning of class.
• FR Initial Application
• Checklist
  • Instructors notify students that copies of ALL items on checklist must be submitted for KY FR Certification at the end of class.
• $10 application fee

Within two (2) weeks of the FR course completion, instructors submit the following:

• Course Summary Report
• Course Summary Roster
• Student Testing Eligibility Form for each student
• Checklist Items- Certification will not be issued without all checklist items.
• Skills Exam Sheets
  • FR Skills Exam
  • FR Skills Exam Evaluation
• $15 money order or cashiers check payable to the Kentucky Board of EMS (for KY FR certification).

No later than July 31, of each year the EMS-TEI Course Coordinator will submit the Annual Summary Report Form.  

If no courses were taught during the last reporting period, file an Annual Summary Report stating that no courses were taught during the reporting period.
