IC Proof of Lecture and Skill Presentations Form (pdf):  If applying for Level I or II EMS Instructor, you must teach five (5) lecture and five (5) skill presentations in a basic training course at the level for which you are applying for.  This must be completed with an instructor who has taught a minimum of three (3) classes as the lead instructor.  If you have any questions, please email amanda.hutson@kctcs.edu or call 859-256-3565.
